Hi team,

I was hoping for some guidance as to how your teams track discharge readiness and how that information is disseminated to the team?

Specifically, does anyone have any good tips for integration with Epic for discharge planning?

Hi @Andrew. Our team has this difficulty as well, We use Epic which has a spot for “Therapy cleared for discharge” but it doesn’t get used. Would love to get examples from hospitals where this is a better process!
