b)     What are the primary patient/family therapy-related goals and how is progress tracked?

c)      What tests/measures are typically conducted by PT (or other disiplines)  in clinic?

e)     Does PT see every patient in the clinic or only patients that meet certain criteria?

f)       Are any of the clinic patients seen as outpatients (at your facility or other community clinics)?  Are there any specific OP referral criteria?
