  1. Do you do pre-implant counseling and audiology work up?  
  2. Do you provide AVT at your hospital?  
  3. Are there other AVT’s in the community (outside the hospital system) where patients can be followed for AVT?  
  4. Are there other CI programs in the community (outside the hospital system) where patients can be seen for audiology and/or AVT?  
  5. Do you dispense hearing aids through your hospital (if you are connected to a university or outside clinic/practice, please indicate)? 
  6. Do you process CI upgrades and repairs through your hospital (vs patients being served directly through the manufacturer)?  
  7. Does Medicaid in your state require a pre authorization for CI? 
  8. If you do not dispense HA or manage CI patients, who in the community do you refer patients to for these services?  
  9. Is your hospital affiliated with a school? If so, what services are offered at the school (AVT, mapping, pre-CI work up, HA, BAHA, etc)?
